Some skin pigmentation is inevitable simply by being exposed to regular unprotected UV rays. Whether your pigmentation is sun induced or a result of hormonal activity (Melasma), genetics or trauma (squeezing spots etc) to the skin, it ages your appearance.
The skin consists of two main layers, the epidermis which is the upper layer and the dermis which is the lower layer. Within the lowest layer of the epidermis are cells called melanocytes, which are responsible for producing the pigment called melanin. This melanin production is what gives our skin it’s genetically predetermined colour and when over stimulated, can produce excess pigmentation.
Melasma is characterised by patchy brown discolouration of the skin, commonly seen on the cheeks, chin, forehead and upper lip in a symmetrical pattern. It primarily occurs in women but can also appear in men. While it can affect all racial and ethnic groups it is more common in darker skinned individuals. With Melasma, the melanocytes become over stimulated and over produce melanin in response to changes in oestrogen and progesterone levels. Pigmented cells then slowly migrate to the surface, making the pigmentation visible.  The most common cause is pregnancy or oral contraceptive use but it can occur for no apparent reason.

Skin Ageing





 What is skin ageing?


We are all destined to experience skin ageing. Skin ageing is a degenerative process caused by intrinsic (chronological) and extrinsic (environmental) ageing.  These factors are something most of us can relate to, and contribute to the visible signs of skin ageing:

• A loss of skin firmness and elasticity

• Fine lines and wrinkles

• Sun damage and pigmentation

• Irregular pigmentation and texture

• Large pores

• Dry skin

• Loss of facial volume

Intrinsic Ageing


Unfortunately, intrinsic ageing is something we have little control over. This is because it is a natural process, determined by internal factors and is programmed by our genes.

Extrinsic ageing, also known as photo-ageing, is caused by external factors that we are regularly exposed to and can be prevented or minimised.  External elements such as sunlight, pollution, nutritional intake, smoking, stress and lack of sleep are among the most common factors. What causes age spots?


Skin ageing is something we will all suffer with as we grow old. It causes the surface layers of our skin (known as the epidermis) to thin, and the number of melanocytes (the pigment producing cells) to decrease. If we have some areas of the skin that are more exposed to the sun, the remaining melanocytes in these areas will increase in size. This leads to our skin appearing thinner and more pale, with large pigmented areas known as age spots, lentigos or liver spots.

What causes skin dryness?


The production of oil slows down, which can cause our skin to become dry, and also our blood vessels become more fragile, which is why we have an increased chance of bruising.

What causes sunken eyes and a ‘skeletal’ appearance?

UV Damage


The sun emits different types of harmful rays, predominantly UVA and UVB. UVB is responsible for burning the upper/outer layers of the skin, whereas UVA penetrates more deeply, and is responsible for causing collagen damage and skin ageing. Most dermatologists advise that the best anti-ageing product you can buy is a good quality sunscreen

The SPF number is a guidance to the level of UVB protection a cream offers, and correlates to the time you can spend in the sun without burning. If your skin type is such that you start to get  red after 15 minutes when unprotected, then wearing a cream with SPF 30 means it would take 30 x 15 minutes (= 7.5 hrs) before redness occurred.However, it is important to reapply the product, as required as efficiency diminishes through sweating and contact/rubbing.

The SPF value (eg 15, 25, 30 etc.) is a guide for the UVB protection, and not the UVA. It is for this reason that broad-spectrum creams are recommended, to also protect against ageing UVA rays. It’s a myth that your moisturiser or foundation which just has a number will provide adequate sun protection. Unless it actually specifies that it provides broad spectrum cover, you may not be getting any protection at all from UVA.



It is also a myth that suncream is only needed in the summer. Whilst UVB rays are at their highest between April and October and between 10am and 4pm, UVA rays are present all year round. I advise wearing a broad spectrum suncream every day, even in winter, from first thing in the morning, and even indoors. This is because UVA rays are unaffected by cloud cover and also penetrate glass.Apply suncream on the areas exposed daily to the elements as damage becomes cululative with every day of exposure.
Medical grade suncreams have been more rigorously tested than over-the-counter ones, and are therefore much more reliable.

Why does our Skin Age ?



Our skin ages as a result of sun, harsh weather, and bad habits. But we can take steps to help our skin stay supple and healthy-looking.

How your skin ages will depend on a variety of factors: your lifestyle, diet, heredity, and other personal habits. For instance, smoking can produce free radicals, once-healthy oxygen molecules that are now overactive and unstable. Free radicals damage cells, leading to, among other things, premature wrinkles.
There are other reasons.
 Primary factors contributing to wrinkled, spotted skin include normal aging, exposure to the sun (photo ageing) and pollution, and loss of subcutaneous support (fatty tissue between your skin and muscle). Other factors that contribute to ageing of the skin include stress, gravity, daily facial movement, obesity, and even the position we sleep in, avoid sleeping on your tummy.


As we grow older, changes like these naturally occur:

Skin becomes rougher.
Skin develops lesions such as benign tumours. Solar keratosis.

Skin becomes slack. The loss of the elastic tissue (elastin) in the skin with age causes the skin to hang loosely.
Skin becomes more transparent. This is caused by thinning of the epidermis (the surface layer of the skin).
Skin becomes more fragile. This is caused by a flattening of the area where the epidermis and dermis (layer of skin under the epidermis) come together.
Skin becomes more easily bruised. This is due to thinner blood vessel walls.
Changes below the skin also become evident as we age.
Loss of fat below the skin in the cheeks, temples, chin, nose, and eye area may result in loosening skin, sunken eyes, and a skeletal appearance.
Bone loss, mostly around the mouth and chin, may become evident after age 60 and cause puckering of the skin around the mouth.
Cartilage loss in the nose causes drooping of the nasal tip and accentuation of the bony structures in the nose.

Sun and Your Skin


Exposure to sunlight is the single biggest culprit in skin ageing.

Over time, the sun's ultraviolet (UV) light damages certain fibres in the skin called elastin. The breakdown of elastin fibres causes the skin to sag, stretch, and lose its ability to snap back after stretching. The skin also bruises and tears more easily and takes longer to heal. So while sun damage may not show when you're young, it will later in life.
Nothing can completely undo sun damage, although the skin can sometimes repair itself. So, it's never too late to begin protecting yourself from sun exposure and skin cancer. You can delay changes associated with ageing by staying out of the sun, covering up, wearing a hat, and making a habit of using sunscreen.
Gravity, facial movement, and sleep positions are the secondary factors that contribute to changes in the skin. When the skin loses its elasticity, gravity causes drooping of the eyebrows and eyelids, looseness and fullness under the cheeks and jaw (jowls and double chin), etc.

Facial movement lines become more visible after the skin starts losing its elasticity (usually as people reach their 30’s and 40’s). Lines may appear horizontally on the forehead, vertically on the skin above the root of the nose, or as small curved lines on the temples, upper cheeks, and around the mouth.
Sleep creases result from the way the head is positioned on the pillow and may become more visible after the skin starts losing its elasticity. Sleep creases are commonly located on the side of the forehead, starting above the eyebrows to the hairline near the temples, as well as on the middle of the cheeks. Sleeping on your back may improve these sleep creases or prevent them from becoming worse.
Smokers tend to have more wrinkles than non-smokers of the same age, complexion, and history of sun exposure.
Dry skin and itching is common in later life. About 85% of older people develop dry skin because overheated indoor air is dry. The loss of oil glands as we age may also worsen dry skin. Anything that further dries the skin (such as overuse of soaps or hot baths and showers) will make the problem worse. Retin-A or Tretinoin is one of the most powerful products in the war against skin ageing.





Retin A is the brand name for Tretinoin - it  has proven itself to reduce, reverse, and prevent wrinkles. (The younger a person starts using it, meaning in their teens and twenties, the more benefits they receive, but it’s never too late to start using it!)
1. Retin A helps lighten and prevent age spots.
2. Retin A builds collagen to thicken and increase the structural strength of treated skin.
3. Retin A helps to decrease a person’s risk of developing skin cancer.
4. Skin just looks better and younger when people use Retin A.

Important advice!

  • Always apply it to your skin at bedtime. 
  • Never use it during the day.
  • Never layer Retin A with benzoyl peroxide and ideally don’t apply it when you also have acid products on your skin like vitamin C, glycolic acid or salicylic acid.
  • Always wait at least 15-30 minutes after cleansing and applying any other skin care products to your skin before you apply Retin A.
  •  Use on its own, do not apply another cream on top. 


Whilst Retin A can have a magic effect on the skin, it can result in some issues if it’s not used correctly. The effect of increased skin cell turnover can be irritation and flaking. For this reason, many people stop using Retin-A after a couple of days to weeks, then think that it didn't work. It is important to realise that Retin-A is very effective for whiteheads and blackheads, but it may take 6-9 weeks to see a noticeable difference. It takes at least 6 months to see a noticeable difference in wrinkles. The best benefit is seen if Retin-A is used for at least a year. When you start to use Retin A, you need to begin with “pearl-sized” not “pea sized” portion for the face, neck and chest.  Use it like it is very precious ‘gold’. It is important to begin using it gradually to allow your skin to adjust. I recommend that clients use it twice a week (every third night) for the first couple of months. During this time, understand that a little peeling (or in my case a lot of peeling) and irritation is normal at first, but if the treated skin becomes really sensitive, back off the Retin A for a bit. Let your skin return to normal before you begin the treatment again. As your skin adjusts, slowly try to increase to using it every night.
Always use a sun protection next day, avoid exposure to direct sunlight.
Stop using Retin A, a month or so prior to going on a sun or ski holiday.
